What we do

As your GP surgery we play a significant role in managing your long-term condition. We believe in working with you and specialists in the community to provide holistic, personal, and high-quality care.

Long term conditions or chronic diseases are conditions for which there is currently no cure, and that are managed with drugs and other treatments.

All patients with a long-term condition should have an annual review (or ‘health MOT’). The first part of this is with a health care assistant or a nurse. At this appointment we will take a blood sample, check your blood pressure, and update other health data.

A GP will review your results, looking at your medical history and medications, and consider how together we could improve your health/management of your LTC. This may include health advice, medication changes or referrals to a community service.

Your second contact with us will be to share and discuss the GP’s recommendations with you. This appointment will also be with a healthcare assistant or a nurse. If you have more than one long term condition, we aim to make this second appointment with a nurse or GP.

Annual Appointments for Long Term Conditions and Blood Tests

We will plan your annual reviews and/or blood test monitoring to coincide with your birthday month.

This system is designed to be simpler for everyone. Simpler for the patient to understand and safer for the Practice to administer. After the first year, it may also reduce the number of annual appointments that patients with long term conditions or who require annual blood tests, need to attend.

Rather than be subject to lots of different dates of reviews throughout the year, you will now be contacted and asked to book your blood test and/or review appointments during your birth month.

Which conditions require an annual health check?

If you have one of the following conditions, then often you are required to attend the Surgery for an annual health check:

  • Diabetes/Pre-diabetes
  • Hypertension
  • Kidney disease
  • Coronary heart disease
  • Asthma
  • COPD
  • Learning disabilities
  • Dementia
  • Routine drug monitoring
  • Heart failure
  • Severe mental health problems

Sometimes you may need an interim review (for example every 3 months, or after 6 months) in which case you will also be asked to book an appointment at this time.

Wherever possible, if you have more than one long term condition, we will try to reduce the number of different appointments you have to attend. However, it is possible that you will still need to attend more than once for your reviews.
